Finnish Home is contracted through the Vancouver Coastal Health Authority to provide residential care for clients with complex care needs. The Health Authority’s responsibility includes the admission process and determining the resident fees.
To have someone admitted to Finnish Home you must go through your local Health Authority.
If someone for whom you provide care (e.g. an aging parent or spouse) needs more help due to chronic illness, disability or dementia, please contact your Health Authority.

The Case Manager from the Health Authority will conduct a thorough standardized assessment of the person’s need and urgency for residential care to determine eligibility for admission.

Hoitolaitokseen hakemisprosessi
Hoitolaitoksellamme on sopimus Terveysministeriön alaosaston kanssa siitä että toimitamme hoitoa asukkaille joilla on monenlaisia terveysongelmia. Terveysministeriön osastolla on vastuu hoitolaitokseen hakuprosessista ja siitä miten paljon asukas joutuu maksamaan hoidosta.
Jos halutaan että joku henkilö muuttaisi asumaan Finnish Home hoito-laitokseen niin pitää ottaa yhteyttä paikalliseen Terveysministeriöön.
Jos joku lähimmaisesi jota sinä hoidat (esim. vanhempasi tai aviopuoliso) tarvitsee enemmän apua pitkäaikaisen sairauden, vammaisuuden tai dementian takia ota yhteyttä Terveysministeriöön.

Niinkutsuttu ”Case Manager” Terveysministeriöstä suorittaa perusteellisen arvioinnin henkilön kunnosta ja siitä miten kiireellisesti hän tarvitsee hoitopaikkaa. Tämä arviointi johtaa siihen että henkilö jolla on suurin tarve on etuoikeutettu ensimmäiseen vapaaseen hoitopaikkaan. Jos sinä tai lähimmäisesi olette sairaalassa ota yhteyttä sairaalan sosiaali työntekijään.
